This is regarding the discussion we had in the last month on the issue from the transitional care scenario for allied health community. One particular complication seen in the field level is centralized information repository is not actually being distributed throughout the organization properly, the information management systems is not effective. In large organizations, where the decisions are decentralized, such lack of sharing of core information is contributing to the inefficiency in the medical care. The top management lethargy to improve the effectiveness of the system, by continuous monitoring, tracking effectiveness and updating the operations for better performance in terms of better care quality, more accurate treatments etc is another issue. In this regard the issue can be resolved by implementing the following policy in the organization.
